From Referral to Results: Our 3-Stage process to creating genuine positive change

Breaking Barriers

Breaking Barriers is our primary program which focuses on our specialty: working directly with youth!

Discover the stages of our unique 10-week program that set Vital Insight apart.
We believe that achieving goals is nearly impossible without understanding why they are important. Similarly, without insight into how barriers are rooted in thoughts and beliefs unrelated to the goal, progress can feel out of reach.

In every interaction, our aim is to help clients challenge limiting values and beliefs, reframe perspectives, and align their outlook with their goals. This is how long-term growth occurs. This is how Vital Insight improves our clients' quality of life permanently.

3 STAGES

Foundations

Stage 1 focuses on the essential groundwork needed to ensure a structured and effective program. Once a Social Worker is available, your referral will be promptly reviewed, and you will receive a Service Agreement outlining the terms of engagement. After completing the Service Agreement, a Social Worker will contact you within 48 hours to arrange your first meeting. During this session, we will collaborate to define and refine your goals. Following this meeting, the Social Worker will develop a comprehensive plan tailored to your needs. This plan will outline key resources, schedules, goals, strategies, potential barriers, and other essential elements to set the foundation for Stage 2.

Growth

Stage 2 is where things become more fun! This is where we play outdoors in the activity most meaningful to the client. This might look like playing a sport, riding a bike (or learning to), throwing a ball back and forth, or simply sitting on a park bench. Conversations will initially be aimed at building rapport. As a positive relationship develops, so too does the challenging reflective/reframing nature of the conversations, providing deeper personal insight for the client. Both the activities and the conversations are aimed to challenge yet move at the pace manageable for the client.

REFLECT

Stage 3 provides an opportunity to reflect and debrief. Conversations encourage the safe space to openly share thoughts and feelings. It is vitally important that each client understands exactly how much they have progressed in their goal. A final report will be compiled to also show tangible change, which will be accessible to all stakeholders of the client’s choosing. This report is equally vital for funding bodies, such as NDIS, to understand ongoing barriers and challenges that may not have been understood previously.

What is Social Work?

How can Social Work help me?

Social Work can help you in various ways, depending on your needs. Social Workers provide counseling, connect you with resources, advocate on your behalf, and assist you in navigating life's challenges.

At Vital Insight we conduct an analysis of your situation, develop a plan and work side-by-side with you to remove barriers and achieve your goals.

Do you accept NDIS clients?

Yes! NDIS participants are funded under the item line:

Improved Daily Living - Capacity Building (15_621_0128_1_3)

Important: Select NDIS as the funding option when completing the Referral Form. This will automatically adjust the form for NDIS clients.

Where do the sessions take place?

Sessions can be held in various locations and is planned by the Social Worker following the first meeting. Depending on the client’s needs, sessions may be held in outdoor areas (parks, basketball courts, soccer fields etc), schools, or other community settings.

In-home visits are arranged if it aligns with the client’s goals and circumstances.

We welcome referrals from everyone, including:

* Clients (Self-referrals)
* Family members
* Child Safety
* NDIS Support Coordinators
* NDIS Plan Managers
* School representatives
* GPs
* Social Workers, Psychologists, & other professionals.
